Stop by Cascade High School on Aug. 19, 5-7:45 p.m. for a free sports physical -- brought to you by Cascade Medical and Cascade School District.
This year, we're scheduling groups of students by last name to help reduce wait time.
5-6 p.m.: Last names A-I
6-7 p.m.: Last names J-R
7-7:45 p.m.: Last names S-Z
Students must be enrolled in Cascade School District. Students who have potentially serious medical conditions will be referred to their primary care provider.
Please bring:
- Gym clothes
- Parents: Please fill out and sign this pre-participation form, examen fisico de pre-participacion
- Parents: These two consent forms will help us keep your student up-to-date on immunization. Medical personnel will look up your student's record and determine which immunizations are due:
