We set aside extra time for your Establish Care appointment so we can understand your health history, current healthcare needs, and goals for the future. This visit helps us build a strong foundation for your care and ensures your care team has the information needed to support you moving forward.
Our Team-Based Approach to Care
Our clinic uses a team-based care model, which means you will have a designated Primary Care Provider (PCP) while a team works together to coordinate your care. To help you get connected and seen sooner, your first appointment may be with one of our Establish Care providers.
If you are hoping to establish with a specific provider, this visit helps ensure they have a thorough understanding of your health history before your first appointment together.
Whether you continue with the provider who completes your Establish Care visit or another provider on your care team, our goal is simple: to make sure you're an established patient with a team that knows you and is ready when you need care.
