The Pantry, 12447 Chapel Drive in Plain
Introducing the new Upper Valley Free Nurse Community Clinic! Starting in August, this walk-in clinic will be open every first Thursday, 4-6 p.m. Terri Weiss, RN, of Upper Valley MEND will provide:
- Surveillance and monitoring of chronic diseases such as diabetes, hypertension, obesity and mental health conditions
- Assessment and triage of non-emergent medical, dental and behavioral health conditions
- Referrals to appropriate medical, dental, behavioral health and alternative providers
- Assist with acquiring medical insurance coverage
