The third annual Free Sports Physical Day is Aug. 19, 5-7:45 p.m., at Cascade High School -- brought to you by Cascade Medical and Cascade School District. Download the forms in advance at cascademedical.org/free-sports-physicals
Students must be enrolled in Cascade School District. Please bring a parent/guardian and wear gym clothes. Students who have potentially serious medical conditions will be referred to their primary care provider.
To help reduce wait times for everyone, we have scheduled groups of students by last name. No one will be turned away, but please plan to arrive at your scheduled time.
- 5-6 p.m.: Last names A-I
- 6-7 p.m.: Last names J-R
- 7-7:45 p.m.: Last names S-Z
